CHAPTER 6.

ROUTINE REPORTS REQUIRED BY THE BUREAU.

6-01. Routine reports are important in the conduct of the bureau's business. The form of report has in each instance been designed to display the required information or data in a concise and logical manner and with a view to simplifying its preparation and insuring that there are no omissions. Since the correct designing of forms of reports is a matter of importance, the bureau desires suggestions at all times as to improvements in the form of the reports, the necessity for new reports, or elimination of any unusual difficulties encountered in their preparation.

6-02. Printed forms for reports. There are listed in this chapter the routine or regular reports required by the bureau, including copies of certain reports submitted to other bureaus. The necessary supply of Yards and Docks printed forms may be had on application to the bureau. In the event that emergencies do not permit obtaining a supply of the forms, yards and stations may submit the necessary report in typewritten form, arranged as indicated by the printed form, but the use of the typewritten form is obviously undesirable and should be avoided.

6-03. Submission of reports. Reports should be submitted promptly and through the proper official channels. It is essential that each officer responsible for initiating the reports keep himself supplied, well in advance of the time of need, with the necessary forms in order that there may be no resulting delay in submitting the reports.

6-04. All reports required are listed and described below and arranged serially under bureau. The principal data in regard to the reports are arranged in suitable type with indentations, so that any particular item may be easily selected. The first line contains the form number and the subject or title of the report, agreeing with the printed title on the form; the second line gives the time. when the report must be submitted; the third line names the officer responsible for initiating the report; the fourth line designates the

group of stations to which the report is applicable; the descriptive matter then follows, at the end of which are stated the destination of the original and copies and the size of the sheets. The arrangement used is indicated by the following outline:

6-05. Officer initiating the report is designated in accordance with the most frequent practice, but owing to the variations in local organizations it has not been possible to do so specifically in certain cases. In such cases the commandant or commanding officer of the yard or station will designate the proper officer.

6-06. The reports required by the bureau are listed as follows:

Y & D form 1. Report of Expenditures Charged to Appropriation "Maintenance, Y. and D."

Submitted monthly.

Public works officer or other officer controlling expenditures. All shore establishments and vessels making expenditures under this appropriation.

The purpose of this form is to furnish to the Bureau of Yards and Docks with early information, in the form necessary for its purposes, as to expenditures charged to its appropriation above named. Report covers all expenditures charged to appropriation "Maintenance, Bureau of Yards and Docks," subdivided under Titles G or S, and E or R, and divided under objects to which applied. Original to the Bureau of Yards and Docks direct; additional copies as required locally. Size of sheet, 8 by 10 inches.

Y & D form 14. Survey of Furniture in Quarters.

Submitted prior to repairs, replacement, or disposal of furniture.
Public works officer or other officer concerned.

Navy yards, air stations, training stations, submarine

bases, and other stations, excepting ordnance stations, radio stations, and hospitals.

The purpose of this form is to enable the bureau to keep a continuous record of furniture in officers' quarters and take proper action on requests for purchases, repairs, or transfers of articles allowed for use in quarters. The form covers the history of the article being reported upon, its condition when surveyed, and a recommendation, which recommendation requires action by the bureau before being carried into effect. Original and one copy to Bureau of Yards and Docks, through official channels; one copy to other bureaus concerned as a matter of information; additional copies as may be required locally. Size of sheet, 8 by 10 inches.

Y&D forms 15 and 15a. Inventory of and Receipt for Furniture.
Prepared when quarters are vacated and submitted when they
are reoccupied; 15a for additional articles.
Public works officer.

Navy yards, air stations, training stations, submarine

bases, and other stations, excepting ordnance stations, radio stations, and hospitals.

The purpose of these forms is to provide and maintain at the bureau, and locally, a continuous record of the furniture in officers' quarters. Form 15 includes the order of the commandant or commanding officer, to be issued when quarters are about to be vacated, that an inventory be made, and indorsements thereon showing action taken, together with the inventory of the furniture. Form 15a covers a list of the furniture, with the history of each article, and at the end the acknowledgment of the vacating occupant and receipt of the new occupant. Form 15a is also to be used, without form 15, when additional articles are placed in quarters between dates of inventories. Original and one copy to Bureau of Yards and Docks, through official channels; one copy to other bureau concerned as a matter of information; one copy to occupant of quarters, and additional copies as may be required locally. Size of sheet 8 by 10 inches.

Y& D form 31. Annual Report of Expenditures and Operations.
Submitted annually.

Public works officer.

Naval districts, navy yards, and naval stations of every class at which any funds under the control of the bureau were expended during the year.

The purpose of this report is to provide and preserve at the bureau, and locally, an annual statement of expenditures made and work done under the cognizance of the bureau. This form is accompanied by instructions in detail for its preparation, which instructions may vary from year to year and should be carefully examined.
