N. B.-Consular officers are instructed to issue the above form in duplicate, and to send the duplicate to the Fifth Auditor, with Form No. 168. The original is to be delivered to the master of the vessel, who must de iver it to the collector of the district in which the vessel first arrives on her return to the United States, and if such master fails to furnish such statement he is liable to a fine of not exceeding fifty dollars.

On this day of before me, the undersigned, personally ap peared United States at, and made oath in due form of law that the above and foregoing report or statement of official services to American vessels and seamen is a true and correct statement of the official services performed by him for the American vessels above named for and during the period of time therein mentioned; that said services were actually and necessarily rendered by him at the instance and request of the masters of the respective vessels herein named; that no services are herein reported except such as were so actually and necessarily rendered in accordance with the provisions of section 12 of the act of Congress approved June 26, 1884, and that the amounts of the respective fees herein stated are in accordance with the tariff of consular fees prescribed by the order of the President of the United States.
