the index finger, within the rectum. The point is now either brought out at the anus, or made to engage against a wooden gorget. If brought out at the anus, the knife is run along the groove, and the tissues severed; and, if not, the knife should be made to impinge against the gorget, by being introduced along the groove of the director, which is then withdrawn. The knife and gorget are now withdrawn together, thus completely severing the intervening tissues; or, a probe-pointed bistoury may be introduced on the director, against the finger, when both finger and bistoury are brought out together. A pair of strong spring scissors may also be used for severing the tissues, being introduced on the director. Great care should be exercised in severing the sphincter muscle-to do it at right angles to its muscular fibres, or at right angles to the anus; for, if divided obliquely, it often fails to unite, and its integrity is consequently impaired.

The after treatment consists in placing a tent of picked lint, or absorbent cotton, anointed with carbolized oil, over the parts. This is to be removed once or twice a day, and the parts cleaned with carbolized water. Healing generally takes place in from two to three weeks. In cases of long standing, where many abscesses have formed and much indurated tissue exists within the sinus, it is well to either incise it freely with the knife or scrape it out with the curette.

being properly washed out and free drainage resorted to, with the patient at rest, may heal without leaving a fistula.

RECAPITUATION OF SURGICAL CASES.

Treated and Operated on in Wards 2 to 8 and 10, Charity Hospital, of New Orleans, La., from October 1, 1885, to March 31, 1886, with Remarks.

The total number of patients, including the out-door patients, treated in those wards during the time mentioned, was 471-all accessible to the students of the medical class, who were not only allowed, but pressed to examine them for themselves, and follow them throughout. Of that number, 65 underwent more or less serious operations-all performed before the class in the operating amphitheatre, after explanation of the

cases.
