A CASE OF AMAUROSIS OF PREGNANCY CONNECTED WITH On Sept. 18th ult., I was called in consultation to visit a woman about thirty years old, of large frame, of stout and vigorous constitution, who was suffering from amaurosis, with profuse epistaxis. She was the mother of two children and now advanced about five months in pregnancy. During the first and second pregnancies she had enjoyed good health with the exception of about one week prior to her second parturition, when she became amblyopic. This condition, however, was transient, ceasing a few weeks after child-birth. No other form of neurosis occurred during either previous gestation, and the parturient act was not marked by any eclamptic phenomena. No observation appears to have been made in regard to the quality of the urine, and hence any conclusion respecting the condition of the kidneys would only be inferential or presumptive. During the present pregnancy she has suffered much from headache, affecting especially the posterior and superior parts of the head. About three weeks ago her right eye became affected with dimness, which condition in a day or two after extended to the left eye. The amblyopia has gradually deepened until she has become almost completely amaurotic. She recognizes no distinction of day and night, and the pupils are largely dilated, and insensible. About a week ago she was attacked with epistaxis from the right nostril which was said to have been quite profuse, and has been followed every day since with a more moderate flow. Previous to this attack her countenance was florid; it is now pallid, and leipothymia follows every attempt to sit erect. Her pulse is about 100, small but somewhat tense. The blood is now pale and thin. The amblyopia has deepened since the epistaxis began, but the cephalalgia has ceased. During the last few days. she has not felt any foetal movements, which were previously active, and they cannot be excited by any abdominal manipulations. Auscultation yields only a negative result. Her appetite is fair, and the quantity of urine voided, she thought, was about natural. No oedema of the face, body or limbs existed. A few days previous to the epistaxis I received a history of her symptoms from her medical adviser, and urged an immediate examination of the urine for albumen; also suggested the free use of the comp. jalap powder, together with a liberal employment of the bromide of potassa with especial reference to the headache. He had failed to detect more than a trace of albumen, but on boiling in a test tube a sample of the urine I found a large deposit of albumen. Tincture of iron, fluid extract of rhatany, and alum whey, had been freely used and with apparent success in restraining the hemorrhage; and believing the foetus was probably dead and the mother's life in danger, several teaspoonful doses of fl. ext. ergot had been given without exciting uterine action. Regarding the condition of the patient as one of considerable gravity, chiefly in consequence of the uremic condition, being apprehensive that if not speedily relieved eclampsia would en |
