HOW TO ORDER PUBLICATIONS

AVAILABILITY OF PUBLICATIONS

Unless otherwise indicated, publications listed in this catalog are distributed by the issuing office which appears immediately before the publication date in each entry. Publications that are sold by GPO are indicated by a "For sale by the Supt. of Docs." statement in the text entries. The GPO stock number (S/N) and the price of the publication at the time the publication was listed in the Monthly Catalog are included in the entry under the item number. (See the sample entry on page v.) Entries for publications that are sold by the National Technical Information Service include a statement to that effect. Additional information necessary for procuring documents appears above or within the text entries. Do not use the Library of Congress card number in ordering publications.

ORDERING DOCUMENTS FROM THE GOVERNMENT PRINTING OFFICE

A. General Information

1. Publications that are sold by the Superintendent of Documents may be ordered directly from the Office of the Superintendent of Documents, U.S. Government Printing Office, Washington, D.C. 20402.

2. Customers may submit orders for publications (please use GPO stock numbers and titles) on the order form found in the front of the catalog. Additional order forms are available from the Superintendent of Documents. Clear photocopies of the order form are acceptable. Instructions for ordering publications and information concerning an order or inquiry, are provided on the back of the order form.

3. The order form may be used to order both publications and subscription services. However, for faster service, we request that customers order subscriptions and publications on separate order forms. Orders for individual issues of subscriptions should be forwarded separately from orders for subscription services. A separate order form for subscriptions is printed in the front of the catalog. 4. Minimum amount of remittance is $1.50.

B. Ordering Publications

1. Order publications by GPO stock number whenever possible. The stock number format is 000-000-00000-0. Include the complete title for each publication requested.

2. If a GPO stock number does not appear in a Monthly Catalog text entry, that means that the publication is not sold by the Government Printing Office and cannot be ordered from the Superintendent of Documents.

3. Stock number, current price, and information regarding stock status is available from the order desk in Washington, D.C. (202-783-3238-a non-FTS commercial number.)

4. Congressional documents and reports that are sold by GPO should be ordered by the stock number and title together with the document or report number and the session of Congress, e.g., Y 1.1/7:96–1. (House document-96th Congress, 1st session; No. 1.)

5. Please indicate clearly on your order if you want the previous edition of a particular publication, and not the newer edition. If you want the latest edition of a publication, indicate this also.

1. Use the latest quarterly edition of Government Periodicals and Subscription Services. Price List 36 or the latest available edition of the GPO Publications Reference File when ordering subscriptions. 2. Always indicate whether the order is for an entire subscription service or for an individual issue. 3. If you are requesting a specific subscription service for the first time, please indicate that it is a new order. If your order is a subscription renewal, return the provided GPO subscription renewal form within 30 days of receipt to avoid a break in service.

4. Order by the complete title of each subscription service and by the Sub List ID when possible (e.g., BGN for Background Notes on Countries of the World) and by stock number. To expedite the processing of your order, include the File Code listed at the end of each subscription entry in Government Periodicals and Subscription Services, Price List 36, e.g., File Code 2M. File codes may be found adjacent to the Sub List ID in the description field of the PRF.

5. Indicate if you desire the service via special postage rates. You may obtain information on postage rates from the order desk (202-783-3238).

6. If you are requesting an individual issue of a subscription service, please provide the complete title, volume number, issue number and date if the issue is a dated (regularly issued) periodical. The Superintendent of Documents retains the latest six issues of most dated periodicals (the latest two issues for most quarterlies) while supplies last.

7. Single copies of some irregularly issued subscriptions are also available for sale. Contact the order desk for availability and price information. Records of individual issues of certain irregular subscription services are also classified on the PRF. Single issues of basic and supplemental subscriptions are not for sale.

8. All subscription services are issued for one year, unless otherwise indicated. On basic and supplemental services, customers will receive all material to date. Customers requesting dated periodicals will recieve the next scheduled issue of the subscription, with the exception of semiannuals and triannuals, in which case the current issue is mailed. customers who order irregular subscription services will receive the next scheduled issue. Most irregular subscription services expire one year after they are entered on the Mail List File. No back material is issued for irregular subscriptions. Back material is furnished for calendar year subscriptions (e.g., Monthly Catalog of U.S. Government Publications, Index Medicus, Code of Federal Regulations, National Library of Medicine Audio Visual Catalog) regardless of when customers submit their orders during the year.

Cancelling a Subscription Service

1. Those subscriptions issued on a regular basis, and a few which are issued irregularly, may be cancelled and a refund made for the unused portion of the subscription. The refund will be prorated based on the number of issues already mailed or scheduled for mailing.

2. There is an $8.00 cancellation charge which will be deducted from the total price of the subscription. 3. If the subscription costs $8.00 or less, no cancellation fee will be assessed and no refunds will be made.

Exceptions:

a) Subscribers who request cancellation of service because the order took longer than six weeks to process, or was processed incorrectly, will receive a full refund regardless of the price of the subscription. No cancellation fee will be charged.

b) Subscribers who request cancellation of service because of a price increase, when the order was processed at the new higher price without their consent, will be refunded the entire amount regardless of the price of the subscription. No cancellation fee will be charged.

4. Customers who request cancellation of service on special six month introductory subscriptions will not be issued refunds or charged a cancellation fee.

5. No refunds will be made or cancellation fees assessed on basic and supplemental subscription services.

Publication orders for domestic customers are shipped non-priority or by United Parcel Service, depending on the zone and weight. Under regular service, domestic publication orders are shipped within five to ten working days of receipt. Publications for international customers are shipped surface mail within five to ten working days of receipt unless the customer specifies and pays for airmail service.

Subscription Orders

Most subscriptions are mailed non-priority unless priority mailing is specified. Allow six weeks processing time plus mailing time for subscription services. Airmail service is available to international customers for most subscription services. However, on basic and supplemental subscription services, only the basic manual and all supplements issued to date are mailed airmail; all subsequent supplemental material is sent surface mail.

E. Ordering via Telephone

1. Customers may place an order by telephone to reduce processing time if the order is to be charged to a VISA, MasterCard, or Superintendent of Documents prepaid deposit account. Customers may call the order desk in Washington, D.C., at (202) 783-3238, between 8:00 a.m. and 4:30 p.m., Eastern time, Monday-Friday, except holidays.

2. Telephone ordering service is also provided by GPO bookstores which carry the more popular publications. (Refer to the list on the back cover for the store in your area.) Customers may order publications by telephone at any GPO bookstore. If the publication requested is available, the order will be processed by the bookstore and the publication will be mailed to the customer. Orders for publications that are not available from the bookstore and orders for subscription services will be forwarded by the bookstore to the GPO Central Office for processing.

3. When ordering by telephone or through the mail, customers should provide an area code and telephone number where they may be reached during the day.

Telex Service

Customers with teletype equipment can also charge orders to a deposit account or credit card account by dialing the GPO TWX #710 822-9413; ANSWER BACK USGPO WSH.

F. How to Remit

Checks

1. All remittances for GPO sales documents should be by check or money order made payable to the Superintendent of Documents. Please do not send currency or coin.

2. With few exceptions, all orders must be accompanied by U.S. dollar remittance.

VISA/MASTERCARD

The Superintendent of Documents is accepting orders to be charged to a valid VISA or MasterCard

account.

1. Customers who wish to use one of these services should furnish their credit card number and the date of expiration.

2. Orders for subscriptions and/or for publications may be charged to your credit card account. Your account will be charged when the item is mailed.
